Catholic Schools Parramatta Diocese (CSPD) is seeking a compassionate, proactive and highly skilled Health Navigator to join our Student Support Directorate on a Full-time Temporary basis for 12 months!

This is a unique opportunity for a professional who is passionate about student wellbeing and enjoys building strong partnerships with families, schools and external service providers to ensure students receive the support they need to thrive both academically and personally.

As a Health Navigator, you will play a critical role in supporting students and families through a collaborative and culturally responsive case management approach. Working closely with school teams, families and community services, you will help connect students with appropriate supports and advocate for their wellbeing and educational needs.

This role is ideal for someone who is passionate about making a positive impact, has strong relationship-building skills and thrives in a collaborative environment.

Key Responsibilities

  • Provide advocacy and support for students and families to access appropriate external health, wellbeing and community services.
  • Develop and implement individualised family support plans with clear goals, measurable outcomes and regular review processes.
  • Coordinate a collaborative case management approach that recognises and respects the unique circumstances and cultural backgrounds of students and families.
  • Establish and maintain strong partnerships with external agencies and service providers to expand support networks for families.
  • Work closely with teachers, wellbeing counsellors, diversity teams and other CSPD staff to deliver coordinated support.
  • Facilitate warm referrals and assist families in navigating complex support systems.
  • Maintain accurate records and monitor progress against agreed support plans.
  • Engage in ongoing professional learning to ensure practice remains current and evidence-based.

About you

To be successful in this role, you will bring:

  • Relevant tertiary qualifications in health, social work, psychology, counselling, community services or a related field.
  • Demonstrated experience in case management, family support, wellbeing services or a similar role.
  • Strong interpersonal and communication skills with the ability to build trust and positive relationships with diverse stakeholders.
  • Experience working collaboratively across multidisciplinary teams.
  • Excellent organisational skills with the ability to manage competing priorities.
  • A genuine passion for supporting children, young people and families.
